Paid Family Medical Leave Resources & FMLA Rights
NCCTK offers paid family medical leave for its employees through Washington State. Paid Family and Medical Leave provides paid time off when a serious health condition prevents you from working, when you need to care for a family member or a new child, or for certain military-related events. The premium is 0.8% of your wage. You may pay about 73% of that total, & your employer pays the rest. This is not including tips and up to the 2023 Social Security cap ($160,200*). STEPS TO TAKE BEFORE SERIOUS HEALTH EVENT:
1.
Email Human Resources about your serious medical event 30 days prior to the event. HR will send you the PFML & FMLA documents below. Print all of the documents.
2.
Schedule a meeting with HR to review the forms.
3.
Fully fill out/sign all of the documents. The documents will need signatures from yourself, HR, your doctor, and spouse (if applicable).
The following forms need to be returned to HR: FMLA Certification of Health Provider, PFML Authorized Rep, PFML Certification of Serious Health Condition, PFML Certification of Birth (if applicable).
4.
Present your doctor with the HR guidance letter.
The following forms need to be returned to HR: FMLA Certification of Health Provider, PFML Authorized Rep, PFML Certification of Serious Health Condition, PFML Certification of Birth (if applicable).
5.
Return forms to HR as soon as possible for approval/denial, processing times, and/or potential payouts. Processing times may vary upon completion of forms.
STEPS TO TAKE AFTER SERIOUS HEALTH EVENT:
1.
Once you have been approved to return to work by your doctor, present HR with a doctors note for details regarding when you can return to work. Make sure that your doctor is as detailed as possible for any PFML leave needed after the serious health event. The doctor must be specific with accommodations. modified hours, etc. The doctors note must go directly to HR and not through the State.
2.
Communicate with HR and your manager regarding your doctor's note and your return to work.

These documents will be sent to you in BambooHR upon approval or denial of eligibility for PMFL. Yourself, your doctor, and and potentially your spouse (if applicable) will all be needed to fill out these forms. Please complete all of the forms as soon as possible, except the certification of birth form. This needs to be given to HR if applicable in event of a birth.
Family Medical Leave Act Documents
These documents will be sent to you in BambooHR upon approval of eligibility for FMLA. Yourself, your doctor, and your spouse (if applicable) will all be needed to fill out these forms. Please complete all of the forms as soon as possible for possible future payments.
